It is important to correctly forward your mail when changing your address to make sure you receive important documents on time. Here are the actions I take when I change my address. Hopefully this will guide you through the process:

To forward your mail you need to submit a Change Of Address (COA) form issued by the Post Office. Make sure to get the correct address and zip code of your new address (unit # if applicable). Gather this information as soon as you know when you are going to move. It is best to fill out the COA form two weeks before you move to avoid delaying time sensitive documents in the mail.
A quick way to submit your COA form is to visit the Post Office website (moversguide.usps.com), although there is a processing fee. When filling out the form, mark the appropriate box that lists permanent or temporary. List the dates of forwarding if temporary and click continue. Mark the type of move (family, business, individual) and hit continue. Provide your email address and continue to the section where you provide your credit card information. It also gives you the option to have a choice of catalogs forwarded to your new address.
Note: To avoid the online charge, click on the “print and mail” tab on the “identity verification” page and print your completed COA form. You can submit your COA form by mail or drop it off at any post office in person.
You can also submit a COA over the phone. Call 1-800-ASK-USPS and provide the same information you would in the online form. This option also charges a processing fee that can be charged to a credit card.
COA forms are available at all post office locations to submit in person at no charge. Find a post office closest to you and submit your COA form at the desk or to any mail carrier.
